Electrical Access Authority Form

Purpose. The network controller and the access recipient complete this form to record the issue, holding and cancellation of an access authority for work on isolated and earthed apparatus.

1.When to use this form

Use this form for every electrical access authority, vicinity authority and test permit. The form is completed during the read back between the network controller and the access recipient and is attached to the work order and the access and permit register entry.

2.Apparatus and limits

Describe exactly the apparatus covered by the authority using the network identification, and the points of isolation and the earths applied that define the limits of the work area. State clearly what remains live adjacent to the work.

3.Issue and read back

The controller reads the authority to the recipient, the recipient repeats it back, and both record the issue time and authority number. The recipient signs to accept responsibility for the safety of all workers under the authority.

4.Workers under the authority

Every worker who works under the authority signs on after the tailgate briefing and signs off when they leave the work area. No worker may work under an authority they have not signed on to.

5.Cancellation

Before cancelling, the recipient confirms all workers are signed off, working earths are removed, and the apparatus is fit for restoration. The cancellation time is recorded by both parties.
