The situation

What has happened

Condemned pole 119 on Buninyong feeder BUN-12, work order WO-26-4410, is to be replaced on Tuesday 16 September with a 22 kV and low voltage outage from 8:00 am to 2:00 pm. Access request EAA-26-2210 is in the register and switching program SP-26-0921 is being checked. The pole is on the Midland Highway shoulder, requiring traffic management, and the low voltage mains supply a dairy that has been told to run its generator. The crew of four includes a second year apprentice, and the elevated work platform and a pole borer are on site. A second 22 kV feeder crosses the road two spans away and remains live.

Your brief. Prepare the crew and site for the outage, take the electrical access authority from the network controller with a correct read back, hold it responsibly for the duration of the work including the working earths and the live adjacent feeder, and cancel it correctly so that restoration can proceed, all in line with the access authority procedure.

What to look for

Evidence guide

The access authority form must define the work area by isolation points and earths and name the live adjacent feeder. Look for a read back recorded at issue and cancellation, every worker signed on and off, working earths applied before work and removed before cancellation, and a risk assessment that addresses the live crossing feeder, the apprentice's supervision and the rescue plan. Authorisations must be checked against the register.
