Purpose. Team leaders use this template to plan the day's task, assess the risks with the crew at the tailgate briefing, record the controls and the rescue plan and confirm every worker's fitness and authorisation.
1.Purpose
The tailgate briefing is where the plan meets the site. It confirms what the crew will do, what could hurt them, how they will control it and how they will rescue each other if it goes wrong. It is completed before every task and repeated when the task, the site or the crew changes.
2.Task and access
Record the work order, the apparatus, the type of access authority or permit required, the access recipient and the switching program number. Confirm the apparatus remaining live and the safe approach distances that apply.
3.Hazards and controls
Walk the site with the crew and list the hazards: electrical, height, plant, excavation, traffic, environment, weather, fire danger, wildlife and fatigue. Record the specific control for each hazard and who is responsible for it.
4.Rescue plan
Record the rescue method for the height or confined work, the rescue equipment on site, the two rescue competent workers, the nearest hospital and the site access for emergency services.
5.Crew sign on
Every crew member confirms their authorisation for the task, declares fitness for work and signs on. Visitors and other contractors are briefed and signed on separately.
